4. Telehealth / Televisit Services
Alight Family may offer telehealth (televisit) services, which are subject to the following conditions:
Telehealth services are provided only after you complete required intake and consent forms through our secure third-party platform (e.g., SimplePractice).
Telehealth services are provided in accordance with applicable state and federal laws.
You must be physically located in Missouri, where the provider is licensed at the time of the televisit.
Telehealth services are not appropriate for all conditions and may be declined if clinically inappropriate.
Detailed telehealth consent, limitations, and technology requirements are provided separately during the intake process.
5. Appointments and Third-Party Platforms
Scheduling, intake forms, billing, payments, and secure communications are handled through third-party platforms, such as SimplePractice.
Your use of those platforms is subject to their own terms and privacy policies. Alight Family is not responsible for the content, availability, or security practices of third-party services.
6. User Responsibilities
By using this Website, you agree that you will:

13. Late Arrivals, Cancellations, and Rescheduling
Late Arrivals
Sessions end at the scheduled time regardless of a late start. Patients arriving more than 10 minutes late may be asked to reschedule. Patients arriving 15 minutes late or more will be asked to reschedule.
Cancellations & Rescheduling
Cancellations or rescheduling require at least 24 hours' advance notice.
The first late cancellation or no-show for an established patient will not incur a fee.
Any subsequent late cancellation or no-show will incur a $100 fee.
A third late cancellation or no-show may result in termination of care.
Missed or late-canceled initial appointments incur a $100 fee, regardless of prior history.
14. No Duty to Respond to Emergencies
Alight Family Integrative Wellness is not a crisis service.
Patients acknowledge and agree that:
Providers are not continuously available
Messages are not monitored in real time
There is no duty to respond to emergencies
Patients agree not to rely on the practice for emergency response
In emergencies, patients are solely responsible for contacting 911, 988, or emergency services.
15. Good Faith Estimate
Under the No Surprises Act, you have the right to receive a Good Faith Estimate explaining how much your healthcare services will cost.
Patients not using insurance are entitled to a Good Faith Estimate. Estimates are non-binding and may vary based on clinical needs. Dispute resolution rights apply if charges exceed estimates by $400 or more.

16. Payment Authorization
By maintaining a card on file, patients authorize charges for services, fees, and outstanding balances. Patients agree not to dispute authorized charges. Unpaid balances may result in suspension of services or referral to collections, with responsibility for associated costs as permitted by law.
17. Notice Of Privacy Practices & Confidentiality
Alight Family Integrative Wellness complies with HIPAA and applicable state and federal privacy laws. Patients have rights to access, amend, and request restrictions on records as permitted by law.
Mandatory disclosures may occur for safety, abuse reporting, court orders, or other legal requirements.
